This client came to Rehab8 after being referred by their doctor for physiotherapy treatment. They were experiencing constant, sharp pain around the upper shoulder blade area, with pain radiating down the outside of the arm. Tingling was also present in the hand, which made the symptoms more concerning and disruptive.

The client was very active and regularly took part in resistance training and endurance workouts. There was no specific traumatic injury, such as a fall or accident. However, they had experienced neck and shoulder-related issues several years earlier.

Because exercise was an important part of the client’s lifestyle, one of the main goals was to help them recover without completely removing the activities that mattered to them.

Initial Findings

During the initial assessment, we found significant weakness with shoulder abduction on the affected side. Other strength tests also showed weakness through the shoulder and arm, including shoulder flexion, biceps, pec major, brachialis, and brachioradialis.

Based on the pain pattern, radiating arm symptoms, tingling, and strength loss, the presentation was consistent with C5-C6 cervical radiculopathy. This was later confirmed through imaging.

Treatment Plan

The treatment plan focused on reducing pain and inflammation, improving neck and upper-back mobility, rebuilding strength, and helping the client continue training safely where possible.

The client worked with us over a longer period and received a combination of treatments, including manual therapy, neck mobilization, electroacupuncture, laser therapy, ultrasound therapy, and progressive exercise rehabilitation.

As symptoms improved, exercises were gradually progressed to restore strength and confidence in the neck, shoulder, and arm.

We also worked closely with the client’s family doctor throughout the process. This included coordinating further imaging and discussing whether conservative or surgical options should be considered.

Results

Progress was gradual but consistent. Over time, the client experienced meaningful improvements in pain, strength, function, and tingling.

By the end of the rehabilitation process, the client had achieved a full recovery. They were pain-free, had regained significant strength in the affected arm and neck region, and had a major reduction in tingling symptoms.

Most importantly, they were able to return to the active lifestyle that was important to them.

This case shows how cervical radiculopathy can create pain, weakness, and nerve symptoms that affect both daily life and training. With a structured physiotherapy plan, medical collaboration, and gradual exercise progression, recovery is possible even when symptoms are significant.
