Client Struggles with Left Knee Pain After Overdoing Walking

A client came to physiotherapy after being referred by their doctor. They had been experiencing soreness in their left knee and noticed a hard, firm bump behind their right knee. Their pain was about 4 out of 10, and they mentioned that when it first started, they couldn’t walk at all. The cause? Simply walking much more than usual during a vacation.

When we assessed them, I noticed that the left knee couldn’t fully straighten, and the quadriceps, hamstrings, and hip abductors on that side were weak. It became clear that this was an overuse injury to the left knee, which had aggravated the meniscus and even caused a Baker’s cyst.

We started gently, focusing on reducing inflammation with hands-on therapy, stretching, and some modalities. The goal was to ease pain while activating the muscles that had weakened. Exercises were prescribed to gradually wake up the quads, hamstrings, and hip muscles to prevent further atrophy.

Physiotherapy sessions were scheduled every two weeks. As the weeks went on, the client’s range of motion improved, and we gradually added strengthening exercises. Hands-on therapy continued, and laser therapy was used to help with inflammation.

After a month, the client reported a noticeable drop in pain, though there was still some tightness. By the second month, their functional strength had improved, the Baker’s cyst had shrunk, and the knee’s full range of motion returned. At that point, we reduced visits to once a month.

By the fourth month, the client no longer had any symptoms. Their strength had improved significantly, mobility was back to normal, and they were discharged.

This case showed how a simple overuse injury can cause a cascade of problems, but with patience, targeted exercises, and consistent physiotherapy service, full recovery is absolutely achievable.
